Consent for Certificate of Professional Conduct
Important Note to Physicians:
Physicians who are closing or leaving their practices have specific obligations with regard to communication and storage/transfer of patient records. Such physicians are strongly urged to consult the following documents:

What is a Certificate of Professional Conduct and what is it used for?
A Certificate of Professional Conduct verifies that a physician is licensed or has been licensed with the College of Physicians and Surgeons of Nova Scotia, and his or her standing with the College.
Medical regulatory authorities/health care organizations in other jurisdictions require a Certificate of Standing / Professional Conduct when a physician applies for a license.
Important Points to Remember When Requesting a Certificate of Professional Conduct:

Read the consent form carefully before signing.

Complete the entire form – an incomplete form will be returned.
- Enclose the fee – payment must be received before the Certificate of Professional Conduct can be issued.
- The completed consent form can be sent to the College of Physicians and Surgeons of Nova Scotia by regular mail or by fax. If sent by fax, payment must be made by Visa, Mastercard, or American Express.
- The usual processing time from the date the College receives your request is five (5) business days. However, processing can take longer in some cases. Please indicate in writing if this is an urgent request, and we will strive to accommodate your request.
- The official (original) copy of the Certificate of Professional Conduct will be sent directly to the organization that has requested the Certificate of Professional Conduct.
How do I obtain a Certificate of Professional Conduct?
1. Complete a
Consent for Certificate of Professional Conduct form (PDF); and
2. If you are currently registered in a postgraduate training program with Dalhousie University AND you hold a full or educational license in Nova Scotia, the fee will be waived if you require the Certificate of Professional Conduct for elective purposes.
Payment can be made by Visa, Mastercard, American Express, cheque or money order and must be submitted with the consent form.
